Revenue up 39%, volume up 53%, prices down 15%, and a guidance range that rose on sales but fell on earnings: the Eli Lilly Q3 2026 results contain several signals that investors and industry analysts will read differently. Reported on 29 October 2026, the quarter delivered $24.4 billion of revenue, with Mounjaro and Zepbound accounting for about two-thirds of it. This financial analysis walks through the price-volume split, the one-time items, the acquired R&D charge behind the lower EPS guidance and what the revised outlook implies for the fourth quarter. It is analysis of reported figures and not investment advice.
Eli Lilly Q3 2026 results: financial takeaways
- Revenue $24,397 million, up 39%; US $15.1 billion (up 34%) and international $9.3 billion (up 47%).
- Volume +53%, price -15%; international price -29%, mainly due to Mounjaro’s inclusion on China’s reimbursement list.
- About $900 million of one-time benefits, including $410 million of Jardiance milestones with none remaining.
- Acquired IPR&D charges of $4.18 billion versus $656 million a year earlier.
- 2026 revenue guidance $91 billion to $92 billion; non-GAAP EPS guidance $35.00 to $36.00.
Price versus volume in the Eli Lilly Q3 2026 results
A 53% volume increase against a 15% price decline means revenue grew 39% only because patient demand more than offset lower average prices. In the US, volume rose 41% and prices fell 7%. Outside the US, volume rose 76% and prices fell 29%. Mounjaro’s international sales of $5.7 billion rose 93%, which suggests that lower-price markets are contributing a growing share of units. The release does not break out the contribution of rebates, discounts and reimbursement changes separately, so the decomposition cannot be taken further. Source figures are in the Lilly release.
Quality of revenue in the Eli Lilly Q3 2026 results
Roughly $900 million of the quarter’s revenue was one-time, including $410 million of Jardiance milestones, which Lilly says leave no further milestones, plus favourable rebate and discount adjustments. By our own calculation, removing all $900 million would leave revenue of about $23.5 billion, still roughly 33% above last year’s $17.6 billion, though that arithmetic treats the whole prior-year base as clean. Mounjaro and Zepbound together generated about $16.1 billion, around 66% of the total by our calculation, so concentration is high.
The EPS puzzle in the Eli Lilly Q3 2026 results
Reported EPS was $7.22, up 16%, and non-GAAP EPS was $7.48, up 7%. Both include $4.57 of acquired IPR&D charges per share, against $0.71 a year earlier, mainly for the AtaiBeckley and Curevo acquisitions. Full-year non-GAAP EPS guidance fell from $35.50 to $36.50 to $35.00 to $36.00. Lilly says underlying guidance rose by $4.07 at the midpoint and that the charges more than offset the rise. That framing is the company’s; investors may treat acquired R&D as a real cost of buying growth.
Eli Lilly Q3 2026 results: what guidance implies for Q4
First-nine-month revenue was $67.17 billion. Subtracting that from the full-year range of $91 billion to $92 billion implies fourth-quarter revenue of about $23.8 billion to $24.8 billion, by our own calculation. Third-quarter revenue of $24.4 billion included about $900 million of one-time benefits, so the guidance range is consistent with roughly flat underlying sequential revenue to modest growth. Lilly also raised its performance margin outlook to 52.0% to 52.5% and its tax-rate range to 19% to 20%, and uses about 894 million shares.
Margins and costs in the Eli Lilly Q3 2026 results
Gross margin was 83.9% of revenue, up 1.0 point. R&D was $4.2 billion, up 23% and 17% of revenue, while marketing, selling and administrative expense was $3.1 billion, up 14%, so costs grew more slowly than revenue. The non-GAAP effective tax rate was 24.0% against 17.7% a year earlier, which weighed on non-GAAP earnings. Net income was $6.45 billion on a reported basis. These are company-reported numbers.
Market context from Acumen Research and Consulting
Acumen Research and Consulting’s Anti-Obesity Drugs Market report valued the market at USD 2.1 billion in 2022 and projected USD 8.2 billion by 2032, a CAGR of 14.6%, with North America above 68% and Asia Pacific growing about 15%. It was last updated in September 2023. Zepbound’s $5.35 billion in a single quarter shows how far actual sales have outrun that forecast, so the report is useful mainly for regional shares (the WHO obesity fact sheet shows the scale of the underlying condition) and as a reminder that market-size estimates date quickly. Questions investors may ask after the Eli Lilly Q3 2026 results
How much further can prices fall before volume growth stops compensating? How concentrated is revenue in two products? How large will acquired R&D charges be next year? When do oral and next-generation medicines contribute meaningfully? Will reimbursement changes in the US and abroad accelerate price declines? The release does not answer these, and we do not speculate.
