The Vertex Q3 2026 results are a test of whether a company that earns about 95% of its revenue from one disease can pay $10 billion to start a second act without losing its earnings power. Vertex Pharmaceuticals reported quarterly revenue of $3.44 billion on 2 November 2026, up 12%, raised its full-year revenue guidance to $13.15 billion to $13.25 billion and absorbed the first month of the Crinetics acquisition. This business analysis looks at revenue mix, margins, cash, guidance and the catalysts ahead, with our own calculations labelled. It is news reporting and not investment advice.
Vertex Q3 2026 results: business takeaways
- Revenue of $3.44 billion beat a consensus estimate of about $3.41 billion, according to a summary of the call; non-GAAP earnings per share of $4.76 were in line.
- Cystic fibrosis revenue of $3.27 billion was about 95% of the total, by our calculation; products outside cystic fibrosis added about $168 million.
- GAAP earnings per share fell to $2.56 from $4.20 on about $502 million of Crinetics-related costs.
- Cash and investments were $9.8 billion, down from $12.3 billion at the end of 2025, after about $9.0 billion of net cash used for Crinetics and buybacks and a $4.5 billion term loan.
- The FDA decision on povetacicept in IgA nephropathy is due on 30 November 2026.
Revenue mix in the Vertex Q3 2026 results
The growth engine is a product transition inside cystic fibrosis. ALYFTREK revenue rose to $706 million from $247 million, while TRIKAFTA/KAFTRIO slipped to $2.44 billion from $2.65 billion. By our calculation, the two combined were $3.14 billion against $2.90 billion a year earlier, an increase of about 8%, which is slower than the 12% total growth. Outside cystic fibrosis, CASGEVY reached $99.2 million, JOURNAVX $58.3 million and PALSONIFY $10.1 million. The company says it expects legacy non-cystic-fibrosis revenue (CASGEVY and JOURNAVX) of $500 million or more in 2026,. By our own arithmetic, the two products totalled about $158 million in the third quarter alone, but the release does not give a year-to-date total here, so we cannot say how close that leaves the company to its target. The full figures are in the company’s results release.
Margins and costs in the Vertex Q3 2026 results
Combined non-GAAP research, acquired research and selling and administration expense rose to $1.53 billion from $1.28 billion, about 19% by our calculation, faster than revenue. A summary of the call puts the non-GAAP operating margin at 41%, down from 45%. Non-GAAP selling and administration expense rose to $556.3 million from $369.0 million, a sign of the commercial build for new launches. The company now expects about $6.0 billion of non-GAAP spending for 2026, up from $5.65 billion to $5.75 billion, including about $250 million for Crinetics operating costs and integration from September to December, according to that summary.
The Crinetics deal in the Vertex Q3 2026 results
Vertex closed the Crinetics acquisition on 1 September 2026 at about $10.0 billion total equity value, or about $9.0 billion net of cash acquired, and recorded about $500 million of compensation and transaction costs. The asset is PALSONIFY (paltusotine) for acromegaly, approved in the US and the EU, with Japan under review and European launches planned for the second half of 2027. September revenue was $10.1 million and 2026 guidance is $40 million to $50 million. At that scale the product does not yet justify the price on near-term sales, which is normal for acquisitions that are underwritten on multi-year growth, but the sources give no peak-sales target, so we do not estimate one.
Balance sheet in the Vertex Q3 2026 results
Cash, cash equivalents and marketable securities were $9.8 billion. Debt was $999.6 million short-term and $3,497.0 million long-term, about $4.5 billion in total by our own addition, which matches the term loan. The share count was 253.5 million. A summary of the call mentions about 250,000 shares bought back for roughly $120 million in the quarter, which we could not confirm in the release. Taken together, the company is using debt and cash to fund an acquisition while still returning capital.
Guidance in the Vertex Q3 2026 results
Revenue guidance moved to $13.15 billion to $13.25 billion from $13.1 billion to $13.2 billion. By our calculation, the midpoint rose $50 million, or 0.4%. It includes a currency benefit of about 1.5 percentage points net of hedging, so part of the raise is not operational. Guidance for combined GAAP research and selling expense moved to $7.15 billion to $7.2 billion from $6.3 billion to $6.45 billion, mostly on Crinetics items. The non-GAAP tax rate is unchanged at 19.5% to 20.5%.
Catalysts after the Vertex Q3 2026 results
The nearest is the povetacicept action date. The drug is under priority review for accelerated approval in IgA nephropathy, a filing based on an interim analysis of the Phase 3 RAINIER trial, and would be the first Vertex nephrology product. A summary of the call quotes the chief executive, Reshma Kewalramani, as saying the company is fully prepared for a launch. Other milestones include interim results from the VX-670 study in myotonic dystrophy in the second half of 2026, inaxaplin interim data in early 2027, atumelnant Phase 3 enrolment completion in the first quarter of 2027 and mid-2027 suzetrigine nerve-pain results. A July 2026 collaboration with AbCellera included a $28 million upfront payment. For another company at an inflection point, see our business analysis of Recursion’s Phase 2 results.
Market context from Acumen Research and Consulting
We did not find an Acumen report on cystic fibrosis or kidney drugs. Two adjacent reports give context. Acumen Research and Consulting’s Gene Editing Market report values that market at USD 18.3 billion in 2024 and projects USD 73.7 billion by 2033, a CAGR of 16.9% for 2025 to 2033, with CRISPR at 42% of 2024 revenue, which bears on CASGEVY. Its Fabry Disease Treatment Market report, a rare-disease example, values that segment at USD 1.7 billion in 2023 and projects USD 3.2 billion by 2032, a CAGR of 7.9%, with enzyme replacement therapy at about 65%. By our own calculation, the stated 7.9% rate would give about USD 3.4 billion over nine years, so the page does not fully reconcile, and it contains a contradictory statement on US approvals. Both reports are indicative and neither covers Vertex products.

GAAP figures include one-off costs such as acquisition expenses and non-GAAP figures remove them. In a quarter with a large deal the two can diverge sharply, as they did here, so it is worth looking at both.
